Senior Health & Community Based Services
Purpose… a joint effort of Public Health Nurses and Social Workers to administer Long Term Care Consultations (LTCC) and Reassessments of Aitkin County residents to determine eligibility for Alternative Care (AC), Community Alternatives for Disabled Individuals (CADI), Community Alternative Care (CAC), and Elderly Waiver (EW), and to connect residents with services such as home health services, home delivered meals, respite care, family counseling/training, special adaptations to the home, medically necessary care, services, therapies, and prescriptions, adult day services, Customized Living (CL), residential care services, adult foster care, and companion services or nursing home placement.
Long Term Care Consultation:
A Long Term Care Consultations is a free evaluation of an Aitkin County resident's current situation, including general health, ability to take care of routine daily activities, home environment, any help received from family and friends, and finances, and, also an evaluation of your actual needs and preferences.
Alternative Care:
Alternative
Care (AC) program is open to eligible applicants age 65 or older, at risk
of nursing home placement, and would become eligible for MA within 180 days
of entering a nursing home. Clients may be eligible for adult day services,
personal care and/or home health services, medical supplies or equipment,
meals on wheels, or Customized Living, residential care services, adult
foster care, or companion services. The monthly cost of home care services
must be less than 75% of the average monthly cost to MA nursing home care.
There may be a fee of 5%, 10% or 30%, dependent upon income and assets.
Community Alternatives Care:
Community Alternative Care (CAC) program is open to eligible applicants under age 65, determined to need a hospital level of care, certified disabled, and eligible for MA; clients may receive medically necessary care, services, therapies, and special services that are necessary to prevent nursing home placement, like respite care, family counseling/training, special adaptations to the home and extended home health services. CAC is offered to prevent hospitalization when the client prefers community services and shows a cost savings to MA.
Community Alternatives for Disabled Individuals:
The Community Alternatives for Disabled Individuals (CADI) program is open to eligible applicants under age 65, certified disabled, eligible for MA and require a nursing home level of care; clients may receive medically necessary care, services, therapies, and special services that are necessary to prevent nursing home placement, like adaptations to the home, extended home health services, and home delivered meals. Expenses cannot exceed the cost to MA for nursing home care.
Elderly Waiver:
The Elderly Waiver (EW) program is open to eligible applicants age 65or older, at risk for nursing home placement, and eligible for MA; clients may be eligible for adult day care, personal care and/or home health nurse services, medical supplies or equipment, home delivered meals, or assisted living, residential care services, adult foster care, or companion services. The monthly cost of home care services must be less than the average monthly cost to MA for nursing home care.
